According to AAHA’s Standards of Accreditation and Dental Care Guidelines for Dogs and Cats, proper documentation of dental procedures is part of the medical record and should include anatomic dental charting, notation of all abnormalities found, and plans for current and future treatment.

Updated in 2015 by a board-certified veterinary dentist, AAHA’s canine and feline dental charts allow your team to record the findings of dental examinations and cleanings, as well as other important patient dental information, in one place.

An open-mouth view of the occlusal surface and numbered tooth illustrations allow for easy charting of periodontal pocket depths, gingivitis levels and fractured or missing teeth.  Fields are also provided for exam findings, recommended and performed treatments or procedures, remarks and diagnosis, radiology evaluation and assessment, and treatment summary and plan. In addition, an extensive legend allows you to note findings in shorthand.
