AI-powered compliance support: The missing leg of the stool

Veterinary medicine has made extraordinary progress in the quality of in-clinic care. Diagnostic capabilities, surgical techniques, and treatment protocols have advanced enormously. Many conditions that were once difficult to manage are now routinely treated, and patients are living longer, healthier lives as a result.
Yet despite these advances, a persistent gap remains in how the profession supports patients and families between appointments—particularly when treatment requires consistent action at home. The gap is not expertise. It is follow-through.
When a client walks out of the clinic with a treatment plan, a prescription, and a handout, the clinical team’s job is technically done. But the patient’s outcome is not yet determined. What happens at home—whether the medication is given correctly, whether the exercises are done, whether early warning signs are noticed—determines whether the care delivered in clinic actually works.
The three-legged stool
The veterinary profession has developed two of three essential supports for successful treatment:
- Affordability: Payment plans, financing options, pet insurance and initiatives like spectrum of care (SoC) have meaningfully expanded access to care.
- In-clinic quality: Standards of care, specialist access, and diagnostic capability continue to advance.
The third leg—compliance, or whether pet owners follow through with treatment at home—remains the least developed, and arguably the most consequential.
The limitations of current follow-up
Historically, post-visit support has been limited to printed instructions and, in some practices, a single follow-up phone call. These tools were designed for a world with different communication habits and different expectations of care continuity.
When follow-up is absent or inadequate, the following can occur:
- Medication errors go undetected until the next appointment.
- Rehabilitation protocols are started with enthusiasm and abandoned within days.
- Clients who are unsure about a step don’t call—they make their best guess or stop the treatment entirely.
- The practice has no visibility into whether the treatment is working until the patient returns, potentially with a worsened condition.

1. The rehabilitation example—where compliance matters most

Post-surgical rehabilitation is among the clearest cases of compliance failure in veterinary medicine. Exercises are demonstrated in the clinic. The patient performs them correctly. The client leaves with a protocol. Within days, without supervision or prompting, the routine falls apart—not from neglect, but from the absence of consistent reinforcement.

Research in behavioral science consistently identifies the same driver: the prompt. Not motivation. Not education. The daily check-in.
An AI system that reaches out each morning (“How did the exercises go? Any signs of swelling or pain?”) and receives and responds to the answer is not replacing the physical therapist’s expertise. It is supplying the nudge that makes that expertise count. This isn’t theoretical. Three days later, the handout is gone and they are not sure about the insulin dose. They do not call—they do not want to bother the clinic for something they feel they should know. The clinic uses an AI-powered communications system that proactively reaches out two days post-diagnosis. The client picks up when the AI-system calls at 7 p.m., and the AI walks them through the protocol with context and patience. The AI is not making a clinical decision. It is delivering the clinical decision the veterinarian already made, at the moment the client needs it.
3. What personalization actually means
There is a meaningful difference between a bulk reminder text and a contextual follow-up conversation. A message that says “Don’t forget Max’s medication” is a nudge. A phone call that asks, “I see Max is on day 4 of his insulin protocol—how is the administration going, and do you have any questions about the dose?” is an extension of the care relationship.
Well-designed AI follow-up is:
- Contextual. It references the specific patient, diagnosis, and treatment plan.
- Bidirectional. The client can respond and receive meaningful answers, not just read a message.
- Escalation-ready. Anything concerning is flagged for clinical review.
- Documented. The exchange is logged in the patient record for continuity of care.

Clients who see their pet recover well become loyal, vocal advocates for the practice.

Compliance failures often present as after-hours crises. Consistent follow-up can catch problems before they escalate.

Clients who feel supported are more likely to complete full treatment plans rather than discontinuing when things become uncertain or inconvenient.
Reframing veterinary success
Success in veterinary care does not end when the client leaves the clinic. Increasingly, it can be defined by:
- Whether the treatment plan was followed correctly at home.
- Whether the client felt supported and confident throughout.
- Whether the practice had visibility into the patient’s progress between appointments.
The stool has three legs. Veterinary medicine has invested heavily in two.
